North Central District Health Department (NCDHD) is seeking a Community Health Manager to provide leadership and oversight for public health initiatives within the Community Health scope of work, which is designed to improve health outcomes through strong community partnerships, system-level approaches, and by integrating staff into the community to provide education and resource navigation. The Community Health program encompasses supervision and development of Community Health Workers (CHWs), coalition work, community health improvement planning, and health education. Efforts support department-wide goals and respond to emerging community needs.

This position is responsible for coordinating daily program operations, establishing and strengthening partnerships across healthcare systems and community organizations, and ensuring effective, compliant, and outcome-driven service metrics are met. The Community Health Manager collaborates with internal leadership and external partners to strengthen referral pathways, oversee budgets and workplans, develop long-term sustainable funding strategies, monitor performance metrics, and advance program growth.

This role also maintains a hands-on component. The manager should have the ability to directly support individuals and families by navigating community services, addressing social determinants of health, reducing barriers to care, and promoting healthy behavior change across the district’s service area.

Education and Experience: Bachelor’s degree in health education, Human Services, Social Services, Sciences, Health Care, Nursing, or related field, required. Previous experience in health coaching, chronic disease management, project management, supervision of staff and data analysis, preferred.
