Job Summary

Community Impact Coordinator works to address the health needs of the community. Work includes helping patients and their families in navigating community services, addressing social determinates of health, addressing cultural or language barriers, and adopting healthy behaviors.

Responsibilities and Duties

  • Serve as a community health worker and assess local support systems’ coordination and deliver services to rural, disabled, and racial and ethnic minority communities including the measurement of health systems resources to help address gaps in access to care, food security, housing, transportation, and other social determinants of health.
  • Create and coordinate a community-based coalition(s) of local community partners that considers local minority health needs.
    • Strive for a membership that serves minorities, will meet at least quarterly to work on assessment, gathering data, strategic planning, implementation of strategies, and evaluation of programming to best serve this population.
  • Utilize data and maintain a robust resource guide that identifies resources relating to community needs that improve health (e.g., access to food, transportation, utility assistance, etc.).
  • Partner with community referrals on initiatives to improve health in the community.
  • Complete trainings on evidence-based programs that improve health of residents in our area.
  • Maintain education on Medicaid access and enrollment while maximizing contact to rural, disabled, and racial and ethnic minority communities.
  • Assist with the Community Health Assessment and Improvement Plan. Utilize social determinants of health data to ensure gaps are being identified and addressed.
  • Assist with NCDHD clinics (wellness, dental, and vaccinations).
  • Support clerical work and utilize these opportunities to connect with residents and help refer patients to other community resources.
  • Promote the health department and its services at community events.
  • Support NCDHD in ensuring access to translation services and assist with translating written materials.
  • Assist with accreditation, specifically with areas that can utilize data to determine community needs and areas that address chronic disease management.
  • Other duties as assigned.

Education and Experience:  

  • Ability and willingness to provide emotional support, encouragement, and motivation to patients.
  • Previous experience in health coaching, chronic disease management, project management, data analysis, preferred.
  • Bachelor’s degree in health education, Human Services, Social Services, Sciences, Health Care, Nursing, or related field, preferred.
  • Fluency in English and Spanish, preferred.

Knowledge, Skills, and Abilities:

  • Strong oral and written communication skills on the phone, virtually, and in person.
  • Attention to detail and good organizational skills that allow an individual to handle multiple priorities while remaining professional and calm.
  • Ability to work compassionately with a diverse array of people and build repour with diverse populations.
  • Ability to maintain effective working relationships with co-workers, providers, the public, and community partners.
  • Computer experience including data entry, MS Word, Excel, PowerPoint, Outlook, required.
  • Ability to operate general office equipment.
  • Understand data analysis and reporting.

Physical Requirements:

This position will sit and stand at a desk for a prolonged period of time while working on a computer. This employee will bend, lift (minimum 30 pounds), and twist on occasion. This position may be required to work in extreme weather conditions.

Special Conditions:

Travel to clinics or community events may require leaving or returning outside of normal business hours. Weekend hours are very limited but may be required for a special event or to respond to a disaster.  Local travel within the 9-county service area is routinely required. Occasional travel to other Nebraska communities for purposes of region-wide or statewide collaboration and training is also required.  (Access to NCDHD vehicle).

Licensure/Certification:

Valid driver’s license and reliable transportation is required.
