Community Access to Coordinated Healthcare, Inc (CATCH) is a non-profit organization dedicated to improving health outcomes through health education, training, outreach and referral services including case management and access to care in four rural public health districts in eastern Nebraska.
CATCH is looking for a Network Director who will oversee and coordinate activities of a Federal four-year rural health network development grant. The network partners include local health departments (LHDs), critical access hospitals, and rural health/physician clinics. North Central District Health Department in O’Neill is one of the participating health departments in this partnership group.
This is a contractual position at $75,000 each year.
Responsibilities:
The Network Director will be responsible for project oversight and will lead the development and implementation of the care coordination structure and interventions, and will assist with training, problem-solving, and quality assurance of the work plan. This position will initiate quality improvement models if an issue arises and will meet with participating LHDs and clinics on a monthly basis either virtually or in-person.
He/she will be responsible for overseeing the project’s work plan and will follow the recommendations of the Board of Directors; coordinate program activities and meetings with committees, workgroups and collaborative partners.
He/she will manage resources and oversee finances to ensure that the project progresses on time and on budget; be responsible for aggregating data collected and prepare progress reports; and make contractual, financial, or other adjustments to align the developing project with the broader outcome goals.
Qualifications:
Prefer previous experience in a leadership position at a healthcare facility or public health organization. MPH and /or MSN preferred.
Will also consider business management experience with strong organization, communication and leadership skills; ability to collaborate with clinicians and community agencies.
A home office is required with working knowledge of common software programs.
Some travel is required throughout the eastern part of Nebraska and to a National meeting. Travel will be reimbursed.
